Onwards and Upwards: Summer Transitions 2024
The Summer Transitions Programme 'Onwards and Upwards' returns in 2024 providing a three day programme for those young people moving from primary/middle/home school to secondary education.
The programme aims to support the young people to feel better prepared for transitioning to secondary education, these young people will likely already be highlighted by education requiring further support with transitioning to secondary school.
The three day programme will run at Windsor Family Hub 10am - 3pm on 27th - 29th August 2024.
There are 16 spaces available on the programme with a minimum 8 required for the programme to run.
Referrals can only be completed by education staff, other professionals can liaise with school/education to ensure a referral is made. All referrals are assessed by the team and will be decided based on individual needs and group dynamics. Referrals will be assessed and agreed on a first come first serve basis with a waiting list in place once full. A maximum of two places will be available per referring school.
